The current recommendation is to give warfarin (INR 2.0-3.0) or LMWH for at least 7-10 days after hip or knee replacement. Several studies shown on this slide have given a LMWH (dalteparin or enoxaparin) for longer periods and shown lower rates of DVT demonstrated by venography. These studies involved relatively small numbers of patients and did not demonstrate that the longer periods of prophylaxis reduced symptomatic DVT.
